Shwaffle's 5e Encounter Sheet


This sheet is intended to make a DM's prep time significantly shorter by compiling all the necessary info needed for a combat encounter in seconds. DM's are able to modify most data after it has populated onto the sheet allowing for any customizing you would like!

I will always have this available for free because I want to help all DM's with their campaign planning. I do this in my free time as a coding hobby. If you feel this tool is worth it, please give it a review and share with your friends! If you love it feel free to throw me some beer money, but you are not obligated to.


  • Manuals to add: Monster Hunter for 5e
  • Create 'lists' like Monster types, terrain, plane etc
  • Add option for the sheet to roll initiative for selected monsters (Will not be adding options for Player characters)
  • Add encounter Challenge Rating based on the monsters selected (Encounter Difficulty)
  • Optimize Attacks and Actions to remove repetative text and make lines overall less wordy
  • Ability to import custom data

  • Hid the encounter difficulty part for the time being, as I have been getting negative reviews and feedback due to it.

v1.61 (7/19/18)

This one is all bug fixes! I highly recommend deleting all previous copies to make sure you're using this version. I would also love to know if this alerts people when I update? Is there any other way I should be alerting people? Let me know.

  • Went through and tested all monsters so none of them should error out. Please report any oddities and bugs on GitHub
  • Changed the verbiage of two-handed variant attacks in the descriptions to make it more apparent
  • Fixed a bug where some attacks wouldn't show based on the ability
  • Small code changes for consistency

v1.6 (7/7/18)

This feels like a small update due to the changelog, but I think the way I reworked how you choose a monster is such a big addition that I wanted to push it out as quickly as possible. This has been something I've wanted to add for quite a while so it feels nice that it's finally finished. This is the groundwork I needed to get the different lists into the sheet so let me know what you think!

Per usual I took too long with me recent update. This is a big one so I hope it makes up for the delay in the new manuals! I would love to have some input from users on what they think needs to be changed or added. After submitting this file, I'm going to begin work in a single page monster sheet. This will be extremely beneficial for boss level monsters that have a large traits/actions section. After that, I would like to work on an initiative order page! That sounds like a fun challange so I hope it turns out well. I appreciate everyones support and thank you for using my sheet!   • Fixed the font size of Speed so it's actually readable.
  • Encounter XP was only pulling the XP from the top monster. I am very sorry for those that relied on this and were given the wrong information.
  • Previous versions of the sheet would disable form highlighting for Adobe globally. This has been fixed to re-enable form highlighting on close.
  • Fixed a lot of errors in Monster Imports


  • Added Tome of Beasts.
  • Added Mordenkainen's Tome of Foes.
  • Added The Tortle Package.
  • Added radio buttons at the top to select whether or not you would like Adobe's field highlighting. I think it looks better off, but choose what you'd like!
  • Added some nice icons for the manuals you select.

He not only pointed me in a great direction to get this completed, he wrote up small section of code to showcase what his sheet does in a simple manner. I downloaded this ages ago, and I'm getting back into things for my wife and daughter. Have the updates mentioned at the beginning of the year been added?

Sadly, no. While this remains a nice static tool for tracking encounters, the part that autofills monster stats is stuck in 2018.
I hope Shwaffle is OK. :-(

Just getting back into the game after a 3 year break. My 13 and 14 yr old sons had me dust off the old 3 and 3.5 books and run them thru the Forge of Fury module. After watching the enjoyment they got from the run i decided to jump back in completely. Found this site 3 days ago. Your encounter sheet is a must anyone planning to run the game. It has cut the time it takes me to prep encounters by 3/4. And with two eager new gamers drooling over my shoulder waiting for their next fix this is exactly what I needed. So from me and on behalf of my sons I would like to say thank you. This is a godsend!

I was looking for a form fillable party tracker for our session 1 of Tomb of Annihilation this weekend. In a bit of a hurry, I found this, scanned the descrip, and downloaded it thinking that it would fit the bill. However, after opening it up I realized what its capable of and I. am. floored. Initially, I didn't know what I had. I lucked-out finding this tool. It is amazing and more useful than I could have dreamed for this huge, complicated TOA campaign I have planned. Thanks to you, Shwaffle! I'm definitely a fan. I'm grateful and impressed. Every DM should have this in the toolbox!

It might just be because I am new to using an Ipad, but I tried using this with the adobe reader Ipad app and I can't get it to let me choose monsters, does anyone have any experience using this on an Ipad? Thanks

Sheet looks awesome...but it doesn't work on my Mac. I even downloaded Adobe Reader for Mac and it still doesn't work. (sigh)

I'm sorry if I'm a total noob, but I've used this form once and loved it, but now that I'm trying to use it again I can't get the monster list to show up. I click on "Choose Monster" after selecting my book, but all it does is clear any information in the monster areas without pulling up a list or any way for me to select a monster type.

What pdf viewer are you using? Make sure you only open it in Adobe Reader. Try downloading a new file and test that, if it happens again let me know

I feel like I have been giving some false promises lately that I haven't been able to keep. (mostly to those that have asked me questions in email)

Mad Mage/Dragon Heist are on my todo, but I am also working on v2 of the sheet which has a lot of QOL updates on the back end for me, but also frees up some real estate on the pages and makes things easier for additions in the future. I really hope everyone likes it, considering I've been teasing it for ages.

Transcribing the monster data is extremely time consuming and I have been working on things as I can. I thank everyone for their continued patience as I balance everything the best I can.

Hi all, would anyone have any advice as to how to get this to work in Linux?
I can read the PDF with Master PDF Editor 5, but I can't seem to auto-fill out or even choose any monsters, it's just blank fields.
To be clear, nothing happens when I click on the "Choose Monster" button.

Hi! Sorry for my late reply, I don't get notifications for messages here.

With the Javascript used within the PDF, Adobe Reader is required. I did some searching and I do not see a Linux compatible reader that supports Javascript. You could try using Wine, but I do not know how if it will work the same as on a standard Windows or Mac operating system.

These products were created by scanning an original printed edition. Most older books are in scanned image format because original digital layout files never existed or were no longer available from the publisher.

For PDF download editions, each page has been run through Optical Character Recognition (OCR) software to attempt to decipher the printed text. The result of this OCR process is placed invisibly behind the picture of each scanned page, to allow for text searching. However, any text in a given book set on a graphical background or in handwritten fonts would most likely not be picked up by the OCR software, and is therefore not searchable. Also, a few larger books may be resampled to fit into the system, and may not have this searchable text background.

For printed books, we have performed high-resolution scans of an original hardcopy of the book. We essentially digitally re-master the book. Unfortunately, the resulting quality of these books is not as high. It's the problem of making a copy of a copy. The text is fine for reading, but illustration work starts to run dark, pixellating and/or losing shades of grey. Moiré patterns may develop in photos. We mark clearly which print titles come from scanned image books so that you can make an informed purchase decision about the quality of what you will receive.
Original electronic format
These ebooks were created from the original electronic layout files, and therefore are fully text searchable. Also, their file size tends to be smaller than scanned image books. Most newer books are in the original electronic format. Both download and print editions of such books should be high quality.
